He is a great man, and the interview was lively, but I did not hear the reasons for the huge cost discrepancy. I could have missed it of course.
I would guess that the lower wages in India account for much of the cost gap.
Also, U.S. hospitals use heart surgery to harvest insurance company dollars and cover other fixed costs. I suspect that the surgery alone does not cost $100,000.
